This occurrence on April 23, 1981 vas notified to your office on May 12,1981 by a telephone call to Ms. Debby Ford.

= d At 1650 on Oh/23/A 1 , the outside operator discovered the drain valve for

  1. 11 Vaste Neutralizing Tank (MIT) open. The operator immediately shut the valve to terminate the discharge. The MIT vas placed in service at 1h20 to receive make-Up regeneration vaste. At approximately 1530 the water treatment specie. list started regenerating #12 make-up train. Between 15h5 and 16k5, approximately 75 gallons of 97% H2S0h which vas diluted to 6%

vith a 27 gallon per cincte dilution flov was discharged to the #11 MC.

Starting at 1630, 50% NaOH, which was diluted to 5% vith a 21 spm ailution flow, was being discharged to the #11 MiT vhen the open drain valve was discovered. It was determined that 50 gallons of 50% tiaOH vere discharged.

Since the MIT drain valve ve.s not shut , all regeneration sent to #11 WT from 15L5 to 1650 vas discharged to the Chesapeake Bay, after being diluted with 1.2 million gpm circulating water flov (discharge 001), without ensuring that the regeneration vaste was in compliance with our diccharge permit.

The cause for the unnonitored discharge was operator failure to follow approved proceduret.. Following the discharge, procedures controlling the MiT vere reviewed and found to be adequate. Operators involved in the occurrence received appropriate disefplinary action. In addition, all operators vill be made aware of thr e,:currence .

The dilution of the accidentally released materials, both sulfuric acid end sodium hydroxide, was so great that no pH shift vould be expected.

The concentrated sulfuric acid was diluted to a 6% solu + ion before the release into the 1.2 million Epm of water and this release occurred ever an hour period. The resultant release of 28.25 gpm into 1.2 million gpm would be diluted, one part in 960,000 parts . This vould result in no pH shift of si gnifi cance .

- Also the sodium hydroxide release of 50 gallons which was diluted to a 5% .

solution occurred into 1.2 million gpm of circulating vater over 20 minutes.

The- 23 5 gym of 5% sodium hydroxide into 1.2 million gp= of water is one part in h80,000 parts dilution. This diluti on vould result in an impercep-tible change in pH. - Therefore , no adverse effects .to the environment are expccted.
