Subject: NRC Region I Inspection No. 50-423/90-10
This letter transmits the report of the routine resident safety inspection con-
ducted by Mr. K. Kolaczyk of this office on June 12 through July 23, 1990, at
Millstone Nuclear Power Station, Unit 3. Waterford, Connecticut. The report
covers activities authorized by NRC License No. NPF-49. The inspection find-
ings were discussed by Mr. Kolaczyk with Mr. Clement and others of your staff
at the conclusion of the inspection.
Areas examined during this inspection are described in the NRC Region I inspec-
tion report which is enclosed with this letter. Within these areas, the in- '
spection consisted of observation of activities, interviews with personnel,
measurements, and document reviews.
Based upon the results of this inspection, it appears that one of your activi-
ties was not conducted in full compliance with NRC requirements, as set forth
in the Notice of Violation enclosed herein as Appendix A and further discussed
- in section 8.3.1 of the enclosed inspection report. The violation concerns an .
inaccurate statement in Licensee Event Report 90-013 regarding the revision of ,
a procedure. A similar inaccuracy had occurred in LER 89-009, dated June 12,
1989, and corrective action was not effective in preventing recurrence. This
violation has been categorized by severity level in accordance with the
" General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR 2 Appendix C (Enforcement Policy 1990). You are required to respond to
this letter and in preparing your response, you should follow the instructions
in Appendix A.

cedures of the Office of Management and Budget as required by the Paperwork
Reduction Act of 1980, PL 96-511.
