This will acknowledge receipt of your. letter dated August 15, 1990, and your check for $25,000 in payment for the civil penalty by the NRC in a letter dated July 16,1990.
In your. letter, you acknowledged that' the violations-occurred as stated. However, you expressed concern that you were..ot' successful in communicating the. thoroughness of your corrective actions to the specific-deficiencies at the May-24, 1990 enforcement conference. Further, you offered to meet with NRC a second time to provide a more comprehensive understanding!of your activities and goals.
We understand your initiatives regarding the programs to update and verify the design configuration of Millstone 1.
However, your initial corrective actions subsequent to identification of the Main Steam Line high flow setpoints and Gas-1 Turbine Generator surveillance testing violations were not considered prompt and comprehensive.- For example, action was not taken at that time to. issue a proce-dure that would formalize the process for identification and: tracking of design issues that could impact the operability of-nuclear safety related' equipment, although such a. procedure had been in development for.over a year. _ Further, prompt action-was not taken to require a' comprehensive review of. previously.
dispositioned design issues recently identified by your safety. system functional inspections and. design-basis reconstruction to determine.by;re-examination whethu.other incorrect engineering determinations had been.made. 1Those actions
'have now been taken in response to your discovery in September 1990 of' the potential for operation of the low pressure coolant injection system in an incorrect configuration. As those actions will be examined during future l
