Electrical and Instrumentation Maintenance The inspector reviewed the licensee's Electrical and Instrumentation Maintenance activities by witnessing work activities, reviewing documentation, and interviewing licensee personnel.

5.1 Procedures Review The licensee's procedures, listed in Attachment 1, were reviewed to ensure written procedures had been established for routine, corrective or emergency maintenance activities and that they included the following:

Administrative approvals for removing the item from service and returning it to service;

Provisions to require that the latest approved drawings are used;

-

Hold points for inspection and signoff by QC;

-

Provisions for returning the affected system to the original configuration following maintenance;

Provisions for operational readiness testing following maintenance;

-

Requirements for special authorization for activities involving welding, open flame, or other ignition sources;

-

Provisions to assure that materials, parts and components are correct and suitable, including environmental qualification;

Provision for assuring that TS limiting conditions for operation are identified and satisfied;

Provisions for the control of housekeeping;

Provisions for obtaining approval from operations;

Provisions for equipment failure analysis and trending;

Provisions for removal of appropriate jumpers, restoration of lifted leads, reinstallation of tubing, and removal of test devices;

Provisions to control work in radiation area No violations or deviations were identifie .2 Witnessing Maintenance Activities The inspector witnessed work activities performed by electricians and I&C technicians. These activities included corrective and preventive maintenance. During these observations the inspector determined the following:

-

Proper operational personnel had been notified and clearances issued;

-

Appropriate maintenance work orders had been issued and approved;

-

The latest approved procedures, drawings and instructions were being used;

-

Properly specified parts and materials were used;

-

QC inspection signoffs were made where specified in the procedures;

-

Qualified test equipment and tools were in use;

-

Adequate identification of jumpers and lifted leads was maintained;

-

Proper restoration of barriers and covers was accomplished;

-

Required system lineups had been accomplished and verified prior to functional testing;

Adequate post maintenance functional testing was performed; l

Fire Protection precautions were observed;

Limiting conditions for operation were met; and,

Housekeeping was satisfactory.

The insp.ector witnessed portions of the following maintenance and surveilipnce activities:

-

GEkks,VACBreakerCompartmentCheckout Breaker compartment preventive maintenance was performed on three cubicles by two electricians who inspected and cleaned breaker contacts, meggered insulation resistance, and checked motor winding continuity using a Wheatstone Bridg Calibration of Core Spray Room Pressure Switch I&C technicians used procedure MI-03-3A to attempt to calibrate a service water header pressure switch for the IB core spray room cooler. They suspended work when they found corrosion on the pressure switch and decided to replace the switc Barksdale Pressure Switch A pressure switch with a crack in the case was replaced by I&C technicians. This pressure switch monitors the nitrogen accumulator pressure in CRD Hydraulic Control Unit 42-31. The inspector witnessed the installation and calibration of this switc Level Indication System Troubleshooting The inspector witnessed the troubleshooting of the two level indicators on the Containment Atmospheric Control System Nitrogen Storage Tank Level Indicators. The two level indicators provide indication of the CAC Nitrogen supply in both control rooms. The technician calibrated the control room indicators which did not correct the problem. He isolated the problem to a suspected faulty level transmitter. He then suspended work for the remainder cf the shift while he requisitioned the necessary part No violations or deviations were identifie .3 Measurement and Test Equipment During the witnessing of these work activities the inspector selected a sample of seven pieces of measurement and test equipment and verified they were within their specified calibration due dat These instruments are listed in attachment 1. The inspector also reviewed the calibration records for these instruments which were maintained by the licensees calibration lab. These records were readily accessible and properly maintained. . 5 5.4 Records Review The inspector reviewed a sample of twenty-five completed work requests, listed in attachment 1, to ensure Nuclear Quality Engineering had reviewed all safety related work requests, appropriate post maintenance testing had been specified, Shift Supervisor approval had been obtained where necessary, and final review sign-offs had been completed. All work requests appeared to be satisfactor No violations or deviations were identifie .5 Quality Control and Quality Assurance The NRC inspector examined the role of the licensee's quality control department during the performance of maintenance activities. The QC supervisor explained that QC inspectors witness all hold points specified in the Maintenance Instructions and on Work Requests. They also perform random monitoring of plant activities. The NRC inspertar reviewed a sample of the QC Random Monitoring Field Reports (listed in Attachment 1) performed during the past four months and verified they included I&C and Electrical maintenance activitie The NRC inspector also reviewed the licensees' Quality Assurance department involvement in the review of I&C and Electrical Maintenance Activities. The NRC inspector reviewed the licensee's tentative schedule of surveillance activities for the months of October, November, and December, and noted it included plans to -

inspect the maintenance program. The NRC inspector also reviewed a sample of completed QA Surveillance Reports (listed in Attachment 1)

and noted that I&C and Electrical Maintenance activities had been reviewe No violations or deviations were identifie .0 Unresolved Items and Inspector Follow-up Items 6.1 (Closed) Unresolved Item 324, 325/84-33-01, No Test Performed or Required to Verify Proper Operation of Motors After Breaker Preventive Maintenance. The inspector reviewed maintenance instruction MI-10-2K2, GE 480 VAC Breaker Compartment Checkout, Rev. 9, January 17, 1985 which was revised by the licensee to include requirements for the operations department to check the operation of equipment following completion of the PM on the breakers. They are required to start the equipment and verify proper operation or, if the equipment cannot be operated at that time, to initiate a Work Request and Authorization to verify and signoff proper operation when the equipment is returned to service. This procedure revision satisfactorily addresses the inspectors concer .

.6 6.2 (Closed) Inspector Followup Item 324, 325/84-33-02, Review Cable Installations for Bend Radius and Unsupported Length. The inspector reviewed an engineering evaluation performed by the licensee which concluded that the as installed unsupported length of these cables did not result in unacceptable stress in the cables. The inspector examined the cables on the identified motor control centers and did not identify any which exceeded the maximum allowable unsupported length. The inspector reviewed the results of a survey performed by the licensee which verified that the bend radius for each size and type of cable was not below the minimum recommended in BSEP Specification 048-001. The inspector also verified that the cables were not in contact with the threaded hanger rods. Based on these reviews and inspections the inspector concluded that the licensee had adequately evaluated the concerns addressed in this ite .3 (Closed) Inspector Follow Item 324, 325/84-33-03, Review of Fuse Size During Preventive Maintenance. The ir.spector reviewed maintenance instruction MI-10-2K2, GE 980 VAC Breaker Compartment Checkout, Rev. 9, January 17, 1985, and verified that it was revised by the licensee to include the requirement to verify fuse size and to inspect fuse clips for damage and tension. The fuse size and fuse holder condition are also required to be recorded on the data sheet. During the witnessing of breaker maintenance

activities the inspector confirmed that these steps were being performe .4 (Closed) Inspector Follow Item 324, 325/84-33-04, Procedure MP-31 Should Require Restoration of Fire Protection / Separation Materials. The inspector reviewed maintenance procedure MP-31, Electrical Cable and Conduit Installation, Identification, and Testing, Rev. 6, April 2, 1985, and verified that instructions had been added to require that any fire protection or separation materials, removed during the performance of MI-31, be restored to the original

! condition. The inspector also reviewed MI-31-1, l Installation of Electrical Cables, Rev. 4, dated November l 26, 1984, and verified instructions had been added to l ensure any covers removed for wire pulls are replaced.

The concern identified is that a back log in the review of completed maintenance work requests would keep the desired information unavailable for input to the trending program for plant equipmen Subsequently the licensee has implemented an Automated Maintenance Management System which electronically stores the work order data including equipment type, malfunction, and corrective actions. This new system allows the data required for trending to be recalled, sorted, and edited more rapidly than with the old system.
