NOTICE OF VIOLATION System Energy Rescurces, In Docket No. 50-416 Grand Gulf License No. NPF-29 During the Nuclear Regulatory Commission (NRC) inspection conducted on November 14 - December 18, 1987, a violation of NRC requirements was identi-fied. The violation involved a failure to follow a procedur In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Technical Specification (TS) 6.8.1 requires that the applicable procedures recomended in Appendix A of Regulatory Guide (RG) 1.33 be established, implemented and maintained. Appendix A of RG 1.33 states that the per-formance of maintenance should be co',ered by written procedures appropri-ate to the circumstance Contrary to the above, the licensee failed to provide the necessary controls to ensure that work and testing were accomplished and plant conditions were maintained in accordance with prescribed requirement Four examples of failure to follow procedures or failure to provide adequate procedures are as follows: On November 30, 1987, inadequate work instructions for re-energizing electrical bus 16AB, which had been de-energized for maintenance activities, resulted in the residual heat removal shutdown cooling suction valve E12-F009 to go to the shut position and a temporary loss of shutdown coolin . On November 11,1987, a 3/4 inch sensing pipe for the A channel narrow and wide range containment post accident monitors was capped which rendered the cantainment pressure monitors inoperabl Although drawings and procedures did not provide for installation of a pipe cap, the local leak rate test procedure implied caps should be installe . On December 3, 1987, while restoring from a residual heat removal containment spray initiation logic system functional test, an operator performed procedure 06-0P-1E12-R-0022 step 5.2.33 prior to completing step 5.2.32, even though a caution note existed, resulting in a Division 2 Engineered Safeguards Features actuatio '8801270233 880120 DR ADOCK 05 g6

i System Energy Resources, In Docket No. 50~416 Grand Gulf License No. NPF-29 On December 12, 1987, an inadequate modification Special Test Instruction 1C11-87-001-05 resulted in the isolation of the instru-ment air supply from the reactor protection system scram pilot valve air headers causing the scram valves to drift open and a subsequent reactor scra This is a Severity l_evel IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, System Energy Resources, Inc. is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, Grand Gulf, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a "Reply to a Notice of Violation" and should include [for each violation]: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieve Where good cause is shown, consideration will be given to extending the response time.
