NOTICE OF VIOLATION Entergy Operations, Inc.

Docket No. 50-416 D>

Grand Gulf License No. NPF-29

.During an NRC inspection conducted on July 21 to August 17. 1990, violations of?

NRC-requirements were identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1990), the violations are listed below:

A.

Technical Specification 6.8.1.a.,

requires that written procedures be i

established, implemented and maintained covering activities recomended in Appendix A of Regulatory Guide 1.33, Revision 2 February - 1978.

Regulatory Guide 1.33, Revision 2, Appendix A, recommends procedures covering the following areas:

Surveillance testing, Equipment Control and i

Maintenance, j

-Surveillance Procedure 06-IC-1C34-M-001, Reactor Vessel Water Level High

- (Level 8) Main Turbice/ Reactor Feed Pump Trip Functional Test, Step 5 20,

- requires a, strip of 'non-conducting tape be placed over electrical terminals 1 through 4 and 7 through 14.

Subsequently, step 5.22, requires that two transmation units be connected to terminals 5 and 4 and terminals-

6 and 4.

. Administrat'.ve. Procedure 01-S-07-1, Control of Work on Plant Equipment and Facilitier., requires that work on permanent plant equipment be authorized.

Administrative Procedure 01-S-06-1, Protective Tagging System, requires 9

smaller tags be used where possible, to prevent obscuring indicators on control boards.

Administrative Procedure 01-S-06-2, Conduct of Operations, step -

6.2.1'.K, states that before performing a procedural step, the operator:

-i should ensure that the-device to be manipulated is the correct device.

Contrary to the above, 1.

Surveillance procedure- 06-IC-1C34-M-uvu, was inadequate in that it requires the use of terminal 4 after it was taped.

The restricted access resulted in an I&C technician grounding 'the Reactor Vessel Feed water control system on July 20, 1990, which resulted in a r

feedwater transient.

On July 30, 1990, a contract maintenance crew leak seal injected Moisture Separator Reheater (MSR) inlet isolation valve B, N11F028B instead of MSR inlet isolation valve A, N11F028A.

This was unauthorized work on MSR inlet isolation valve B.

Administrative-Procedure 01-S-06-l'was, inadequate in'that the red tag used-obstructed the operator's view.1 This resulted in the operator-inserting _ a' tagged IRM.o No provisions were; provided for using protective covers on push button type _ switches to prevent obstructing-the operator's view of the control board.

4 On August 16', 1990, a control room operator racked-out the wrong Residual Heat Removal (RHR) pump breaker.

This resulted in having two divisions of RHR inoperable.

This is a Severity Level IV violation-(Supplement.1).

B.

10CFR 50, Appendix B, Criteria XVI, requires that measures shall be estab-

i lished to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies -deviations, and defective material and equip-ment are promptly identified and corrected.

Contrary to the above, On April. 29,1989, the licensee failed to take-adequate corrective 1 action when they identified a control power fuse

. failure problem with the inboard MSIV leakage control subsystem A, an ESF l

system.

The fuse failure made subsystem A inoperable on a MSIV fast l

closure.

The fuse failure was repeated on December 31, 1909 and July 27, l

1990. The fuse problem was corrected on July 29, 1990.

This is a: Severity Level IV violation (Supplement I).
