ENCLOSURE 1 NOTICE OF VIOLATION System Energy Resources, Inc.

Docket No. 50-416 Grand Gulf License No. NPF-29 During the Nuclear Regulatory Comission (NRC) inspection conducted on December 14-17, 1987, a violation of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:

10 CFR 50.59(a)(1) states that the holder of a license authorizing operation of a production or utilization facility may conduct tests and experiments not described in the safety analysis report, without prior comission approval, unless the proposed change, test or experiment involves an unreviewed safety question.

10 CFR 50.59(b)(1) states that the licensee shall also maintain records of tests and experiments pursuant to paragraph (a) of this section.

These records must include a written safety evaluation which provides the bases for the detemination that the change, test or experiment does not involve an unreviewed safety question.

Contrary to the above, a chemical cleaning of loops A and C of the Standby Service Water System was performed during December 1987 without an adequately prepared and reviewed safety evaluation in that weld materials and crevice regions within the system piping were not evaluated for potential degradation and subsequent reduction of margin of sabtv during and as a result of the chemical cleaning, and without adequate records to support the safety evaluation.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, System Energy Resources, Inc. is hereby required to submit a written statement or explanation to the Nuclear Regulatory Comission, ATTN:

Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice.

This reply should be clearly marked as a "Reply to a Notice of Violation" e3d should include :

(1) admission or denial of the violation, t

(2) the reason for the violation if admitted, (3) the corrective steps which t

have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance f

will be achieved.

Where good cause is shown, consideration will be given to i

extending the response time.
