During the Nuclear Regulatory Commission (NRC) inspection conducted on June 16

]

through July 14, 1989, violations of NRC requirements were identified.

In 1

accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1989), the violations are listed below:

A.

Technical Specification 6.8.1.a states, that written procedures shall be established, implemented and maintained covering the procedures recon nended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1989.

Regulatory Guide 1.33 states, safety-rela O d activities should be covered by written procedures for startup, operation and shutdown of safety-related BWR systems; the Reactor Core Isolation Cooling system (RCIC).

System Operating Instruction (S01) 04-1-01-E51-1, RCIC System, Step 4.1, requires the completion of a manual valve lineup checksheet and the opening of the RCIC steam supply valves in a prescribed sequence.

Contrary to the above, on June 20, 1989, plant operators failed to implement the RCIC System SUI when returning the system to a standby condition following maintenance.

This resulted in a RCIC isolation and a Reactor Water Cleanup System isolation.

This is a Severity Level IV violation (Supplement I).

B.

10 CFR 50, Appendix B, Criterion XVI, Corrective Action states, measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment and nonconfonnances are promptly identified and corrected.

Contrary to the above, on July 1, 1989, the A Residual Feat Removal System strainer clogged with suppression pool sediment.

The clogging also occurred in March 1988.

In June 1988, the Plant Safety Review Committee recommended that suppression pool sludge be removed at the end of the next outage. The recommendation was not implemented.

(4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.

If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
