ENCLOSURE 1 NOTICE OF VIOLATION Entergy Operat Nns, Inc.

Docket No.:

50-416 Grand Gulf License No.: NPF-29 During the Nuclear Regulatory Commission (NRC) inspection conducted on October 24 through November 27, 1993, a violation of NRC requirements was identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

Technical Specification 6.8.1.c requires that written procedures be established, implemented, and maintained covering the applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2.

Regulatory Guide 1.33 recommends that administrative procedures for equipment control (locking and tagging) be established.

Administrative procedure 01-S-06-1, Protective Tagging System, step 6.3.2.a, requires the individual performing tag installation shall "...

follow any special instructions as written in the Special Instructions Block."

f Contrary to the above, on October 7, 1993, operators hanging clearances 933749 and 933750 failed to adequately verify inverter lY96 was being supplied from its normal power supply, as required in th; clearance's special instructions, before opening the alternate supply breaker.

This is a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Entergy Operations, Inc. is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, Attn: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC i

Resident Inspector, Grand Gulf Nuclear Station within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a

" Reply to a Notice of Violation" and should include:

(1) the reason for the violation, or if contested, the basis for disputing the violation; (2) the corrective steps ' hat have been taken and the results achieved; (3) the corrective step. cnat will be taken to avoid further violations; and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order or demand for information may be issued as to why the license should not be modified, suspended, or revoked or why such other action as may be propar should not be taken. Where good cause is shown, consideration will be given to extending the response time.
