ENCLOSURE 1 NOTICE OF VIOLATION Entergy Operations, Inc. Docket No.: 50-416 Grand Gulf Nuclear Station License No.: NPF-29 During an NRC inspection conducted on April 5 through May 16,1990, two violations of NRC requirements were identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violations are listed below:

A. Technical Specification 5.4.1.a states, in part, that written procedures shall be implemented covering the applicable procedures recommended in Appendix A of  !

Regulatory Guide 1.33, " Quality Assurance Program Requirements (Operations),"

Revision 2, February 1978.  !

Regulatory Guide 1.33, Appendix A, Section 3.s.(2)(a), recommends procedures for startup, operations, and shutdown of emergency power sources.

Procedure 04-1-01-P75-1, " Standby Diesel Generator System," Revision 50, Paragraph 3.33, requirer, that if the diese! generator control cabinet doors are left open or unlatched, they must not be left unattended. Otherwise, the respective diesel  ;

generator should be declared inoperable.

Contrary to the above, on April 21,1998, the inspectors found the back doors on Diesel

Generator Control Cabinet 1H22-P400 in the Division 1 standby diesel generator room i standing wide open and unattended and the Division i diesel was not declared  !

inoperable This is a Severity Level IV violation (Supplement 1) (50-416/9805-02). l i

B. 10 CFR Part 50, Appendix B, Criterion V, states, in part, that activities affecting quality l shall be prescribed by documented procedures and drawings of a type appropriate to  !

the circumstances and shall be accomplished in accordance with these procedures and l drawings.

Procedure 15-S-01-106, " Scaffold Erection," Revision 0, Paragraph 6.2.2, requires that scaffolding built in a Seismic Category I building, including the auxiliary building, or in the vicinity of safety-related equipment be assembled in accordance with l

Standard GGNS-CS-05. Paragraph 7.4 requires that an independent inspection be  ;

completed to ensure all Standard GGNS-CS-05 requirements have been met. I Standard GGNS-CS-05," Standard for Erection of Scaffolding in Safety-Related Areas,"

Revision 0, Paragraph 4.2.8, requires that scaffolding, its bracing, and ladders be kept a minimum of 6-inches away from all safety-related systems or components. Any encroachment on this separation requires nuclear plant engineering approval.

l Contrary to the above, on April 8,1990, Scaffold C9-1881 was assembled so that it l contacted safety-related reactor core isolation cooling system instrument tubing. l Nuclear plant engineering approval was not requested, no evaluation was performed,  !

and the independent verification was not performed.

Pursuant to the provisions of 10 CFR 2.201, Entergy Operations, Inc. is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission i ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional l Administrator, Region IV,611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy I to the NRC Reaident inspector at the facility that is the subject of this Notice, within 30 days of i the date of the letter transmitting thir Notice of Violation (Notice). This reply should be clearly ,

marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the baris for disputing the violation or severity level, (2) the correct;ve steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violaticns, and (4) the date when full compliance will be )

achieved. Your response may reference or include previous docketed correspondence, if the  !

correspondence adequately addresses the required response. If an adequate reply is not  !

received within the time specified in this Notice, an order or a Demand fu information may be I issued as to why the license should not be modified, suspended, or revoked, or why such other i action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.

If you contest this enforcement action, you should also provide a copy of your response to the Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001.
