ENCLOSURE 1 NOTICE OF VIOLATION Mississippi Power and Light Company Docket No. 50-416 Grand Gulf License No. NPF-29 During the Nuclear Regulatory Commission (NRC) inspection conducted on

, September 8 - October 17, 1986, violations of NRC requirements were identified.

The violations involved failures to provide adequate, documented procedures as required by Technical Specification 6.8.1. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violations are listed below:

A. Technical Specification 6.8.1 requires that written procedures be established, implemented, and maintained covering the activities recommended in Appendix A of Regulatory Guide (RG) 1.33, Revision 2, February 1978. RG 1.33 recommends procedures for performing maintenance repair, replacement and modification work. Section 6.1.2 of Administrative Procedure 01-S-07-1, Revision 17, Control of Work on Plant Equipment and Facilities, requires maintenance work to be accomplished using written and approved work documents.

Contrary to the above:

1. On September 17, 1986, licensee contract personnel modified the refueling platform hoist by removing the grapple air hose restraining clamp without an approved procedure or work request, then proceeded with fuel movement using the refueling platform while in a modified unapproved condition.
2. On September 17, 1986, while performing a modification, inadequate instructions in Maintenance Work Order (MW0) 57689 resulted in lifting an electrical lead from relay K110A, thus deenergizing relay K145A.

This caused an inadvertent ESF actuation, Division 1 secondary contain-ment isolation, and start of standby gas treatment system and control room fresh air isolation.

3. On October 8,1986, while testing the tachometer on Division 1 Standby Diesel Generator, inadequate instructions in MWO E66738 resulted in an inadvertent ESF actuation of the Standby Service Water "A" system.

This is a Severity Level IV violation (Supplement I).

B. 10 CFR 50.72(b)(2)(ii) states that any event or condition that results in manual or automatic actuation of any ESF is reportable to the NRC within four hours. -

Mississippi Power and Light Company 2 Docket No. 50-416 Grand Gulf License No. NPF-29 Contrary to the above, an Essential Safety Feature actuation of the Standby Service Water system on October 8, 1986 was not reported to the NRC within four hours.

Pursuant to the provisions of 10 CFR 2.201, Mississippi Power and Light Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including for each violation): (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.
