i ENCLOSURE 1 NOTICE OF VIOLATION System Energy Resources, Inc.

Docket No. 50-416 Grand Gulf License No. NPF-29 During the Nuclear Regulatory Commission (HRC) inspection conducted on April 15 through May 19, 1989, violations of NRC requirements were identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2 Appendix C' (1988), the violation are listed below:

A.

Technical Specification (TS) 6.8.1 requires that written procedures be established, implemented and maintained covering the activities recommended in Regulatory Guide (R.G.) 1.33, Revision 2, February 1978.

R.G.1.33 recommends procedures for control of radioactivity.

Radiation Protection Procedure Procedure 08-S-01-21, Radiological Practices for.

Controlled Areas, states that all radiological postings. signs and barriers will be strictly complied with and will not be moved or bypassed unless specifically authorized by health physics, and requires that radiation work permits be followed.

Contrary to the above on April 24, 1989, contract carpenters removed a contamination area boundary on the southeast side of the turbine generator without receiving health physics authorization, or meeting the protective clothing requirements specified for that work area.

This is a Severity Level IV violation (Supplement I).

B.

lichnical Specification 6.8.1.a requires that written procedures be estabMsh, implemented and maintained covering applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Regulatory Guide 1.33, Revision 2, Appendix A states that instructions for energizing, filling, venting, draining, startup, shutdown, and changing modes of operation should be prepared, as appropriate, for the feedwater system.

Contrary to the above, System Operating Instruction 04-1-01-N21-1, Feedwater System, was inadequate for the operation of the reactor feedwater pump turbine manual speed changer in that, the manual speed changer must be lowered to the low speed stop before the feedwater pump governor control valve will open.

This inadequate procedure contributed to the loss of feed water control which resulted in a reactor scram en May 5, 1989.

This is a severity level IV violation (Supplement I).

2 Docket No. 50-416 Grand Gulf License No. NPF-29 Pursuant to the provisions of 10 CFR 2.201, System Energy Resources, Inc. is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN:

Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector within 30 days of the date of the letter transmitting this Notice.

This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.

If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
