ENCLOSURE 1 NOTICE OF VIOLATION Mississippi Power and Light Company Docket No. 50-416 Grand Gulf License No. NPF-29 During the Nuclear Regulatory Commission (NRC) inspection conducted on October 18 -

November 14, 1986, a violation of NRC requirements was identified. The violation i involved failure to follow procedures or failure to provide adequate procedures.

In accordance with the " General Statement of Policy for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1985), the violation is listed below:

Technical Specification (TS) 6.8.1 requires that the applicable procedures recommended in Appendix A of Regulatory Guide (RG) 1.33 be established, implemented and maintained. Appendix A of RC 1.33 states that the performance of j maintenance should be covered by written procedures appropriate to circumstances.

Contrary to the above, the licensee failed to provide the necessary program of controls, appropriate to the current refueling outage, to ensure that work and testing was accomplished, and plant conditions were maintained, in accordance with prescribed requirements. Five examples of where the licensee failed to follow procedures or failed to provide adequate procedures are as follows:

1. On October 15, 1986, a clearance for a Division 2 electrical outage did not recognize system conditions existing at the time or specify the necessary tagout sequence to preclude an inadvertent Engineered Safety Feature (ESF) actuation. When the tagout was implemented, an inadvertent initiation of the Standby Gas Treatment System and Control Room Standby Fresh Air System resulted.
2. On October 17, 1986, inadequate work instructions in support of Maintenance Work Order (MWO) P57626 resulted in an inadvertent ESF actuation. When power was restored to Bus 16AB after work completion, an inadvertence Division 2 isolation signal was generated, closing shutdown cooling isolation valve E12-F009.
3. On October 18, 1986, prerequisite plant conditions were not established as required by step 4.4 of surveillance procedure 06-ME-1B21-R-0008, MSRV Operability Test, which required air to the Main Steam Relief Valve (MSRV) solenoids to be depressurized. This resulted in MSRV B21-F041C being inadvertently stroked dry during the solenoid operability test (Click Test) prescribed in step 5.10 of the procedure.
4. On November 6,1986, inadequate work instructions accompanying MWO E64279 resulted in an inadvertent ESF actuation. While replacing an Agastat relay, drywell purge compressor A inadvertently started.

5. Surveillance Procedure 06-1C-SC85C85-0-1003 was inadequate in that reinstallation instructions did not ensure that strong motion accelerometers were aligned within one degree, as required by GGNS Specification 9645-J-701.0.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Mississippi Power and Light Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
