m ENCLOSURE 1 NOTICE OF VIOLATION Mississippi Power and Light Company Cocket No. 50-416 Grand Gulf
!.icense No. NPF-29 During the Nuclear Regulatory Commission (NRC) inspection conducted on July 15 -
August 8,1986, a violation of NRC requirements was identified. The violation involved two examples of procedural noncompliance.
In accordance with the
" General Statement of Policy and Procedure for NRC Enforce nent Actions,"
10 CFR Part 2, Appendix C (1985), the violation is listed below:
Technical Specification 6.8.1 requires that written procedures be established, implemented and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide (RG) 1.33, Revision 2, February 1978, and the surveillance and test activities of safety related equipment.
RG 1.33 states that safety related system procedures should include instructions for the Standby Liquid Control Systems (SLCS).
SLCS Operating Instruction 04-1-01-C41-1 requires pressure instrument root isolation valve C41-FX001 to be in the open position.
Surveillance Procedure (SP) 06-0P-1P75-M-0002, Standby Diesel Generator (SDG) 12 Functional Test, ' states to place SDG in the maintenance mode, simulta-neously press the remote and the local maintenance mode select pushbuttons.
Contrary to the above, during power operation: (1) on July 29, 1986, valve C41-FX001 was found in the shut position, rending the local and remote SLCS pump discharge pressure instruments inoperable; (2) On July 11, 1986, while performing SP 06-0P-IP75-M-0002, the control room operator erroneously pressed the SDG 12 start pushbutton in lieu of the maintenance mode select pushbutton resulting in the inadvertent start of SDG 12.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Mississippi Power and Light is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including (for each violation):
(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given te extending the response time.
