Dear Mr. Keppler:

On the afternoon of December 9,1986, Iowa Electric Light and Power Company received information regarding qualification of nylon insulated electrical butt splices. This equipment is witMn the scope of the Duane Arnold Energy Center (DAEC) Equipment Qualification (EQ) program and was previously considered to be qualified based on test data, analysis and similarity.

Several days earlier, we had become aware that f ailures of nylon-insulated butt splices had occurred recently during qualification tests conducted by a testing lab for another utility. Based upon

, information which we had received orally, we had concluded that the qualification test conditions for these components were more severe than those required for the DAEC environmental envelope and, therefore, that the test f ailures were not necessarily relevant to the qualification of butt splices installed at the DAEC. However, on December 9,1986, we received orally information which indicated that, due to an error at the testing facility, the irradiation of the test specimens was, in fact, less conservative than the environmental envelope for the DAEC EQ program. This information was imediately reviewed by Iowa Electric management and a plant shutdown was initiated. It was our intention that the plant was to remain in a shutdown condition until the electrical splices in question could be qualified by repair or replacement.

s Decenber 19, 1986 NG-86-4544 Page Two The scope of the work to be performed during the shutdown was initially limited to replacement of the nylon-insulated butt splices in the drywell penetration boxes with nuclear grade AMP butt splices. The scope of.the work has now been expanded because of information received from the NRC subsequent to the plant shutdown.

A question exists regarding the qualification of the nuclear grade butt splices in a steam environment; therefore, we are applying a seal on all butt splices within the scope of the EQ program which are located in potential high energy line break (HELB) areas. The seal is applied by covering the splices with an insulating tape to prevent moisture intrusion.

The environmentally qualified taping process is described in Attachment 1.

The qualification of the tape used for the seals and justification for use of the tape seals is contained in Attachment 2. The nylon insulated butt splices located in the drywell penetration boxes were replaced with the r:sclear grade butt splices and a tape seal was then applied. Other butt sclices located in potential HELB areas were not replaced, but a tape seal was applied to these splices whether they were of the nylan or nuclear grade insulating jacket design.

The concerns related to the use of the AMP butt splices are primarily related to moisture intrusion, which is not a concern for splices located in the non-HELB areas. However, there is some question as to whether the butt splices meet the criteria of IEEE-323-1974. Iowa Electric will continue to review this issue and will upgrade any deficient or questionable butt splices to meet those criteria during the next refueling outage. That outage is expected to begin in mid-March, 1987. Justification for not taping the butt splices in non-HELB areas during the interim period is contained in Attachment 3.
