Dear Mr. Davis:
In the letter referenced above, Iowa Electric agreed to submit copies of National Pollution Discharge Elimination System (NPDES) discharge permit !
changes and notices of violations within a reasonable time after they are :
provided to or received from the permitting agency. However, we recently ;
discovered that we had failed to notify the NRC of NPDES permit violations that ,
occurred in April 1985; January through August 1987; and January and February i 1988. These ytolations relate to outfall 002, the discharge from the Extended !
Aeration Sewage Treatment Plant at the Duane Arnold Energy Center (DAEC). A r copy of the month'v operating report for each of these months is attached and r sumtrs ized in Table 1. Values which exceed permit limits are highlighted.
In April 1985 and February through August 1987, refueling outages were j in progress at the DAEC Ond larger-than-usual numbers of people were at the 3 site. The NPDES permit v;olations during these periods occured primarily i because the capacity of the sewage treatment plant was exceeded. The violation in January 1987 resulted from the planned disposal of a discontinued chemical. l The violations in January and February 1988 resulted from the combination of j increased loadings on the sewage treatment plant and cold water temperature, i Also attached are copies of Reports of Investigation by the Iowa Department of Natural Resources, Environmental Protection Division for their 3 inspections of the.DAEC on March 6,1987 and March 18, 1988. [
Iowa Electric is presently completing the censtruction of a new sewage treatment plant which has approximately twice the capacity of the old plant.
This facility was placed in use on October 3,1988. The initial discharges from :
this facility may exceed the permit limits until the necessary biomass is !
established. This is normal for a new sewage treatment plant. l

Dear Mr. Rattenborg:
Please find enclosed the January, 1987, operation reports for
]. Cooling Tower Blowdown, outfall 001, and Extended Aeration Sewage Treat-
, ment Plant, outfall 002. As we notified you by phone, the Sewage Treat-ment Plant has exceeded the pemit effluent limitation for both maximum and average BOD concentrations.
j The cause of this event is the pre-planned disposal 'of a discon-tinued chemical. The chemical was processed through the Sewage Treatment l Plant. When this processing was started, it was noted thn the effluent 2
characteristics had changed significantly. Processing of the chemical was
- imediately suspended. Sample analysis found that 800 concentration had exceeded the limits. It is noted that the effluent BOD on a mass basis (maximum of 8.9 pounds per day) did remain within pemit limits of 13 lb/ day average and 19 lb/ day maximum.
1 No further processing of this chemical is planned. Before con-sidering such disposal of any new chemical in the future, we will deter-i mine the BOD and C00. Should you have any questions, please feel free to contact myself or Wendell Keith at (319) 851-7296.

The Sewage Treatment Plant has exceeded its effluent permit limitations for maximum and average BOD. This can be attributed to a significant increase in loading due to additional personnel at DAEC for a refueling outage. We have increased sludge removal from the plant in an effort to maintain campliance.

Plea. find enclosed the March,1987, operation reports for Cooling Tower Blowdown, outfall 001, and Extended Aeration Sewage Treatment Plant, outfall 002. The Sewage Treatment Plant has exceeded its effluent permit limita-tions for maximum and average BOD concentration and average BOD mass flow rate. This can be attributed to a significant increase in loading due to additional personnel at DAEC for a refueling outage. We continue with more frequent sludge removal from the plant in an effort to maintain comoltance. Concerning your investigation recort of March 18, 1987, we expect to respond by April 30, 1987. Please find enclosed the June, 1987, operation reports for Cooling Tower Blowdown, outfall 001, and Extended Aeration Sewage Treatment Plant, outfall 002. The Sewage Treatment Plant has exceeded its effluent permit limita-tions for maximum and average 800 concentration. This can be attributed . to increased loading due to additional personnel at Ouane Arnold Energy Center for a refueling outage. This outage was completed on June 30, lo87. It is expected that July site population and loading will return to non-- tage levels. It is noted that the 800 mass flow rate was maintained within NPDES limitations during June. Please find enclosed the July, 1987, operation reports for Cooling Tower Blowdown, outfall 001, and Extended Aeration Sewage Treatment Plant, outfall 002. The Sewage Treatment Plant has exceeded its effluent permit limitations for maximum and average BOD concentration. Although the influent flow has returned to typical non-outage rates, increased influent BOD concentration continues to keep the plant at or above the peak design loading. We are presently studying this chance in concentration in an effort to identify the source. It is noted that the effluent BOD mass flow rate was significantly less than NPDES limitstioM during "uly. This can be attributed to influent flows which are higher than typical non-outage flows as well as very cold water temperatures. Please find enclosed the February, 1988, operation reports for Cooling Tower Blowdown, outfall 001, and Extended Aeration Sewage Treatment Plant, outfall 002. The Sewage Treatment Plant has exceeded its effluent permit limitations for BOD concentrations and average suspended solids concentration. This can be attributed to influent flows which are higher than typical non-outage flows as well as very cold water temperatures. It is noted that the BOD and suspended solids mass flow rates were maintained within permit limitations during the month. Below is a list of observations made and ec=ents received during the course of this inspection: (1) There are two permitted discharges frem this facility. Outfall 001 censists of discharge frem ccoling tower bicwdown frem the recirculating water system. Outfall 002 consists of discharge frem an extended aeration treatment plant used to treat demestic. wastewater. (2) The discharge from Outfall 001 consists of river water taken from the Cedar River that passes through the large cooling tower system. The water dis-charges into a small canal on company property, that leads back to the Cedar River, i=.ediately downstream frem the ccmpany's intake building. On this date, the water being discharged frem the cooling systeaa appeared to be clear. No water samples were taken. Monthly operatj sn reports are being submitted for this cutfall. (3) The ce=pany's most recent NPCES permit, issued June 9,1986, requires that
=ine and chremium%e tested frem the water being discharged through Cutfall 001. This menicoring was begun in September of 1986. It is noted that there were no results for =inc and chremium in the December 1986, monitor-ing report. Mr. Keith stated that this was due to revisions that were being made with the analytical equipment at the Duane Arnold Energy Center, and that the samples were missed for that month. Mr. Keith further stated that the samples would continue in the future.

- at this time would be within NPDES permit limitations. >
(5) The domestic waste treatment plant experienced an upset in January, when . six barrels of lignosulfonate, a discontinued chemical used in the cooling towers, were disposed at a rate of two barrels por day through the extended aeration treatment plant. Upon disposing of this chemical, it was discovered that the system was organically overloaded. Sampling results for January indicate violations of the BCD limitationi (6) Mr. Keith explained that the treatment system continues to be organically , overloaded due to .an extraordinarily high number of outside workers that have been on-site recently. Once every two years, this facility experiences a scheduled refueling outage, during which time outsido contractors ccme in and are i wolved in major maintenance at this facility. presently 400 - 600 people are on-site for training purposes in preparation for the upccming refueling outage that is scheduled to start March 15th and will last two months. It is reported that Howard R. Green Engineering Co. has recently ccmpleted an upgrade study, to assess various options available to Duane Arnold Energy Center for the proper treatment of the increased waste frem these temporary workers. Mr. Keith stated that the company is in the process of analy::ing the options 1.nat will enable them to expand the treatment capabilities of this system. The possible options include another temporary package plant being brought on-site, or a waste stabili::ation lagoon being con.structed. (7) A & T Consultants, consisting of Thomas Adams and Larry Trout, have been operating the package aeration treatment system as contract operators, since April 1, 1983. It is Mr. Keith's understanding that the general operation of the treatment plant includes hauling sludge on a ence or twice-per-month basis. It is his understanding that with the upcoming refueling outage shutdown, that A & T Consultants plans on hauling sludge on a once or twice-per-week basis. It is hoped that this will improve the quality of the treated effluent. (8) Mr. Keith stated that the ecmpany is planning on installing chlorination equipment and a ecmposite sa.pler for the package plant in the near future. (9) The primary contact person for this facility in the future will be Wendell Keith. The secondary contact person will be Greg Parker, Chemistry Coordinator. The third possible contact is Lou Kriege, Assistant chemistry Coordinator. . CCNCI,USIONS & RECCMMENDATICNS: The discharge frem outfall 001, consisting of the cooling tower blowdown, appears to be causing no pecblems for this facility. Past operation reports show that the effluent can and does meet the requirements listed on the NPCES permit. It is the ccmpany's responsibility to t.ake sure that all the required s.

Below is a list of obser-vations made and comments received during the course of this inves._gation: (1) There are two permitted discharges from this facility. Outfall 001 consists of the discharge of cooling tower blowdown from the recirculating water system. Outfall 002 consists of discharge from an extended aeration treatment plant, used to treat domestic wastewater. (2) The discharge from 001 on this date was essentially clear. No problems were noted. No water sample was taken. Monthly operation reports are being submitted for this outfall. (3) The discharge from outfall 002 appeared to have a brownish tinge on this date. There were some obvious solids being discharged. Sludge was observed to be bulking at the end of the package plant juct prior to discharge. No sample was taken. Monthly operation reports are being submitted for this outfall. (4) Outfall 002 has been experiencing problems since the previous summer during a scheduled refueling outage. Because of this, our Department is requiring the company to construct a facility that will be able to consistently meet the permit requirements. It is reported that Shive-Hattery has been contracted to design and install a new sequencing batch reactor system, which is to be built north of the present system. , f

Construction is to begin in late spring, with a completion date set for September 1, 1988. The next refueling outage is sched'21ed for the end of September.
CONCLUSIONS & RECOMMENDATIONS: It appears that the discharge water from Outfall 001 consistently meets the permit limitations, as required by the company's NPDES permit. Outfall 002 has had numerous violations of both P0D and total suspended solids throughout the past 12 months. It appears that the company is taking appropriate measures to correct this problem. The company should keep this office informed as to the progress ooing made on the new treatment system. DAH:mc O O l}}
