Nature Of AnLNRC inspector identified 12 electrical Deficiency: 1 separation interactions on two conduits in the Fuel-HandlingLBuilding during a site inspection in. July 1986. To determine if these cases'were isolated a re-inspection of.the Fuel Handling Building elevation 286' and portions of'the Reactor-Auxiliary Building was performed. The results of-these re-inspections indicated that a number of-separation interactions remained after programmatic controls for engineering,. construction, and inspection had-been exercised.

Date Problem: On August 22, 1986,.CP&L (Mr. Kumar Hate') notified Reported: the NRC (Mr. P. Fredrickson) that this item was reportable per the provisions of 10CFR50.55(e).

Scope of This deficiency affected plant' areas with safety Problem: related electrical raceway , cable and equipment.

Safety The separation interactions identified by the Implications: initial inspections were evaluated (including the 12 that were identified by the NRC) and determined not to be safety significant. However, following the initial inspection, the process for identifying interactions included work that had not been previcusly inspected, making it difficult to evaluate the overall safety significance.

Reason If left uncorrected, a separation interaction Deficiency is could cause loss of a safety function or Reportable: redundant feature as a result of an internal fault within a cable.

Corrective Areas that contain safety related raceway, cable, Action: and equipment have been walked down to identify the remaining separation interactions. As required, these items were reworked and re-inspected.

Corrective To reduce the' complexity of installation, new Action: '

electrical design in safety related areas are being (Continued) pre-engineered. Project drawing CAR-2166-B-060 has been revised to reflect the separation criteria previously identified in HPES Guideline 7.5.Q.

Craft supervision has received additional training to the new separation criteria. Increased emphasis and accountability has been placed on craft supervision for installations involving electrical )

separation. Also other discipline work procedures I have been changed to reflect their understanding of separation requirements.

Construction inspection personnel have received additional training to the new separation criteria. Inspection procedures have been reviewed and revised as necessary to enhance the

-identification and documentation of separation

, violations. Installations rejected by construction l

inspection for electrical separation deficiencies are being trended separately to provide visibility to management.

Although adequate corrective action has been taken, it is understood that work which may affect

[ electrical separation will continue. QA l surveillance of work and inspection is an on going

program and it is probable that additional i separation interactions may be identified. Any I future separation problems will be handled in l accordance with existing project procedures.

Final Report: With the completion of the corrective actions as stated above, this item is considered resolved.
