Anacon A DIVtSION OF HIGH VOLTAGE ENGsNEERING CORPORATION July 14, 1986 Director Office of Inspection and Enforcement U.S. Nuclear Regulatory Commission Washington, D.C.
20555 Ref: Anacon Chlorine Probes Gentlemen:
An Anacon commercial customer recently returned to us for modification a non-nuclear system which had never been in service. Upon examination of the probes three of a total of thirty-two exhibited a blue.tip rather than the normal white tip and were found to be insensitive to chlorine. We further ascertained that improper handling and/or storage of the probes were responsible for the losc of sensitivity. The probes had never been in service, but in storage in excess of eight months. The probes were ob-viously mishandled and showed physical damage.
if the customers follow the required procedure of checking the probe for chlorine sensitivity prior to installation, the loss of sensitivity will be easily discovered.
We do not believe that this is an incident reportable under 10 CFR 21; never the less, we consider it appropriate to acquaint you with it.
We have put out an Advisory Bulletin, copy attached, and have sent it via Certified fail to all our Chlorine Probe users. To date we have received no further evidence of this occurence and therefore we consider it an isolated case.
Should we encounter any problems on nuclear installations, we shall im-mediately advise the commission.
Sincerely yours, ANACON p4h Gerald E. Stackhouse President

P/N 150002 - Commercial C12 P/N 140000 - hcl and HF probes P/N 170002 - Nuclear Chlorine We have recently discovered that user mishandling or improper storage of probes used in the Anacon Ambient Air Monitoring System can result in a loss of sensitivity to the measured gas.
This loss of sensitivity is indicated by the formation of a blue color on the probe's sensing tip.
This tip should normally be white in color.
All users should check all probes in use, and in stock, to determine if the probe exhibits the blue color indicating loss of sensitivity.
Probes showing this indication should be taken out of service or storage and returned to Anacon for replacement.
Please contact Nancy Wall at Anacon (617-481-7888) for return authorization.
We wish to remind you that:
1)
All prcbes must be kept in an upright condition at all times.
2)
In storage, they should be maintained in an upright position with the sensing tip down, and should be checked on a monthly basis to verify that the tip has not exhibited the characteristic blue color indicating loss of sensitivity.
3)
Probes in service should be inspected and checked for electrolyte level and gas sensitivity on a monthly basis.
All questions or inquiries relating to probe sensitivity should be referred to Richard Reiter at Anacon, phone 617-481-7888.
