Shearon Harris Nuclear Power Plant, Unit No. 1, 10CFR 50.55 (e) reportable item. A quantity of electrical separation interactions remain undetected after acceptance by installation inspection.

Item: Safety related raceway, cable and equipment.

Supplied by: Not a vendor deficiency.

Nature Of An NRC inspector identified 12 electrical Deficiency: separation interactions on two conduits in the Fuel Handling Building during a site inspection in July 1986. To determine if these cases were isolated a re-inspection of the Fuel Handling Building elevation 286' and portions of the Reactor Auxiliary Building was performed. The results of these re-inspections indicated that a number of separation interactions remained after programmatic controls for engineering, construction, and inspection had been exercised.

Scope of This deficiency affects plant areas with safety Problem: related electrical raceway , cable and equipment.

Safety The separation interactions that have been Implications: evaluated (including the 12 that were identified by the NRC) were determined not to be safety significant. However, the number of existing interactions retaaining in the plant (some yet to be identified) make it difficult to evaluate the safety significance on a timely basis.

Reason If left uncorrected, a separation interaction Deficiency is could cause loss of a safety function or Reportable: redundant feature as a result of an internal fault within a cable.

  • emaining separation interactions. As required, these items will be reworked and re-inspected.

To reduce the complexity of installation, new electrical design in safety related. areas will be -

pre-engineered. Project drawing CAR-2166-B-060 will be revised to reflect the separation criteria previously identified in HPES Guideline 7.5.Q.

Craft supervision will receive additional training to the new separation criteria. Increased emphasis and accountability will be placed on craft supervision for installations involving electrical separation. Also other discipline work procedures will be changed to reflect their understanding of separation requirements.

Construction inspectica personnel will receive additional training to the new separation criteria.

Inspection procedures will be reviewed and revised as necessary to enhance the identification and documentation of separation violations. Installations rejected for electrical separation deficiencies will be trended separately to provide visibility to management.

Final Report: The engineering identification of separation items in the field is on going at this time. Until the scope of field work has been determined, a completion date can not be established. It is currently projected that CP&L will be able to provide a schedule for a final report by October 3, 1986.
