On May 8, 1998, with the plant at approximately 100% power in Mode 1, a surveillance testing deficiency related to preventive maintenance and inspections for molded case circuit breakers was determined to be reportable. Specifically, during a review performed by Harris Plant maintenance and engineering personnel, a concern was raised regarding Technical Specification surveillance requirement 4.8.4.1.b, which states that each containment penetration overcurrent protection device shall be demonstrated operable: "At least once per 60 months by subjecting each circuit breaker to an inspection and preventive m'aintenance in accordance with procedures prepared in conjunction with its manufacturer's recommendations." Procedures that were credited for satisfying this requirement did not include provisions for the 60 month inspection and preventive maintenance (PM) for 480 volt molded case circuit breakers. Per the manufacturer, the 480 volt molded case breakers should have been cycled once to satisfy the inspection and PM requirement. A review of maintenance records indicates that each applicable molded case circuit breaker was not cycled as required. This review identified the failure to cycle nine pressurizer heater bank 480 volt molded case circuit breakers during the 60 month testing interval.

This condition was caused by inadequate surveillance procedures, which was due to confusion related to the actual manufacturer's recommendations for the 480 volt breakers.

Immediate corrective actions included testing the nine pressurizer heater bank breakers by cycling them each. No problems were encountered during this testing. Additional actions will include procedure revisions to ensure proper future testing of the 480 volt molded case circuit breakers.

EVENT DESCRIPTION:

On May 8, 1998, with the plant at approximately 100% power in Mode 1, a surveillance testing deficiency related to preventive maintenance and inspections for molded case circuit breakers was determined to be reportable. Specifically, during a review performed by Harris Plant maintenance and engineering personnel, a concern was raised regarding Technical Specification (TS) surveillance requirement 4.8.4.1.b. Maintenance Surveillance Test (MST-E0007) "120 Volt Molded Case Circuit Breaker" and Maintenance Surveillance Test (MST-E0006) "480 Volt Molded Case Circuit Breaker Test" did not include provisions for the 60 month inspection and preventive maintenance (PM) for the 480 volt molded case circuit breakers. Per the breaker manufacturer, the 480 volt breakers should have been cycled once every 60 months to satisfy the inspection and PM requirement. A review of maintenance records indicates that each applicable molded case circuit breaker was not cycled as required. This review identified the failure to cycle nine pressurizer heater bank 480 volt molded case circuit breakers during the 60 month testing interval.

Upon confirmation of this testing deficiency, Operations was notified and the plant entered TS 4.0.3 at 0834 hours0.00965 days <br />0.232 hours <br />0.00138 weeks <br />3.17337e-4 months <br /> on May 8, 1998. This allowed 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> to complete the appropriate surveillance testing. Testing was completed by cycling the nine subject breakers and TS 4.0.3 was exited at 1145 hours0.0133 days <br />0.318 hours <br />0.00189 weeks <br />4.356725e-4 months <br />.

This condition constitutes operation prohibited by plant TS and is being reported per 10 CFR50.73.a.2.i.B.

CAUSE:

This condition was caused by inadequate surveillance procedures. MST-E0006 did not include specific guidance to ensure completion of the 60-month preventive maintenance and inspections for the 480 volt molded case circuit breakers. This procedure deficiency was caused by confusion related to the actual manufacturer's recommendations for the 480 volt breakers. A probable cause for this confusion was mis-application of guidance provided for the 120/208 volt molded case circuit breakers in 1992. This guidance was in the form a Technical Support Memorandum (TMM-92-192) and a previous interpretation of Technical Specifications, which is no longer active, and was based on manufacturer's recommendations. It stated that no preventive maintenance or inspections were required to satisfy the 60-month TS requirement for the 120/208 volt molded case circuit breakers. However, this guidance did not address, nor was it intended for the 480 volt breakers. It appears that this guidance intended only for the 120/208 volt breakers was improperly applied to the 480 volt breakers, causing the TS violation.

SAFETY CONSE UENCES:

There were no actual safety consequences as a result of this testing deficiency condition. There have been no instances where containment penetration overcurrent protection was required for the nine pressurizer heater bank breakers. Testing performed on the pressurizer heater bank breakers on May 8, 1998, revealed no problems with the breakers.

This condition constitutes operation prohibited by plant TS and is being reported per 10 CFR50.73.a.2.i.B.

PREVIOUS SIMILAR EVENTS:

There have been no previous LERs submitted specifically related to containment penetration overcurrent protection circuit breakers. Several HNP LERs (94-01, 95-01, 95-03, 95-07, 96-02, 97-08, 97-11, 98-05) have been submitted to the NRC and were caused by Technical Specification Surveillance testing deficiencies. A comprehensive surveillance test review project is currently in progress in conjunction with HNP's conversion to the new Westinghouse MERITS Standard Technical Specifications (NUREG-1431) to help resolve these TS testing problems.

CORRECTIVE ACTIONS PLANNED:

1. Maintenance Surveillance Test procedures will be revised or generated as appropriate to ensure that vendor recommended preventive maintenance and inspections are performed in the future to fully satisfy TS 4.8.4.1.b.

This will be completed by September 30, 1998.
