OCG-9190). The purpose of this report is to advise of further identification of affected heads by serial number and recommended corrective action.

The root cause for the failure at G"If States Utilities Company (GSU) was due to inadequate cast wall thickness at the 3/4"-10 bolt hole indicated on the sketch supplied with the 10 CFR Part 21 notification. Our Design Engineering has reviewed the design drawings to determine if any other subcover bolt holes do not have adequate wall thickness between the bolt hole and water passage. Their review revealed no other bolt hole with inadequate wall thickness.

The cylinder heads are identified by one of the following part numbers: 03-360-03-OF, 1 A 6446,1 A-6447,1 A-6879,1 A-6240,1 A-6239,1 A-7062,1 A-5559,1 A-5558,1 A-6266, or 1 A-6265. Cylinder heads affected are those cast prior to August 1,1984.

Identification of individual heads may be accomplished by referring to the attached sketch and from the following information. All heads have the cast serial number, heat number, cast date, manufacturing date, water test, air test, gas test, and possibly reconditioning information stamped on the head. At Area 1 on attached sketch, which is inside the subcover, the serial number, heat number and cast date should be stamped with minimum of 1/4" size letters. Area 2 on the attached sketch, which is outslua the subcover on the head,.;.ludes serial number, heat number, manufacturing date, water test, air test, gas test t Mmation and possibly reconditioning data.

Area 1 should be examined for the cast date as ctamped by the Enterprise foundry with steel stamps. This may be difficult to see as this area has been painted. Other clues to the age of the cylinder tiead are at Area 2. If the serial number is L99 or less, and the manufacturing date is before 1986, it is most likely a cylinder head cast prior to August 1, 1984. , December 17,1992 Corrective action at the affected utility sites may be accomplished by removing the stud at the location shown on the sketch, and applying Loctite Hydraulic Sealant (Item No.

56931) or equivalent, per Loctite instructions to the threaded ! solo and stud. Tests at Cooper Bessemer conducted on December 7,1992, proved this repair sufficient to withstand 100 PSI of water pressure (hot or cold) without leakage. Service Information Memo (SIM) #384 is being issued which explains this procedure in more detail. This corrective action should be performed at the next scheduled refueling outage, or cold shutdown, or whichever occurs first.

Corrective action at Cooper Bessemer will be to repair this head section on all cylinder heads cast prior to August 1,1984 which are retumed to C-B for rebuild or repair with our standard repair EAR 11-001. This involves drilling the hole 1/8" oversize and 1/8" deeper, welding the area complete, redrilling and retapping to blueprint size and depth, and re-hydrotesting.
