Addendum i

Dear Sir,

This report is issued to provide additional inf rmation to eup-plement our Report #145 issued April 29, 1988 to your office.

In our report dated April 29, 1988, we identified a potential prob-lem with the F-573-156 Pressure Sensor / Lube Oil Trip (Calcon P/N B4400).

Additional testing and examinatione conducted by the manufacturer of this component (California Controle Company -

Calcon) have now identif$ed that because of a tolerance etack-up, the diaphragm (Calcon P/N 283) can be held eolid against the pressure head (Calcon P/N 4370), thus creating a emaller eurface area.

This smaller surface area would require a much higher pressure to activate the valve.

On a newly assembled unit thic would not chou up on teet because the diaphragm has not had a chance to etretch and seat itself against the pressure head. Sev-eral houre are needed between teste for the unit to fail. Correc tive action for thie device le to add an additional 1-1/8" diame-ter with.030" deep counterbore in the presoure head (Calcon P/N 4370),

This will always provide a positive gap between the head and diaphragm.

Devices that are already installed and operating after several houre between teste have demonstrated their reliab-ility.

Imo Delaval recommende that all devices not installed, or that are installed but have not operated for several hours between tecte, be returned to Imo Delaval for remachining, increction and testing.

The devices will be remachined, inspected and tested by the manufacturer (Calcon) and identified with the stamping "B4400B" to record the new configuration. IMO Delaval has sup-plied DSR and DSRV engines with thie potential defect to the fol-lowing sites:

Asterisk indicates that the F-573-156 sensor / trip is performing a safety related function in this control system.

A copy of this letter will be sent to all of the sites referenced in the paragraph above as indicated by the carbon copy list, l

Our evaluation of this matter was concluded on May 10, 1988.

Sincerely.
