Dear Sir:
On October 29, 1991 Cooper Industries notified the Commission of a potential defect in a component of a DSRV-16-4 Standby diesel Generator System.
Specifically, there is a potential problem with the driven gear for the Engine-Driven Jacket Water Pump.
We noted in that letter, that gear wear induced failures had occurred at approximately 1,500 operating hours.
The purpose of this letter is to inform the Commission that one of the affected (ENTERGY, Grand Gulf Nuclear Station) has observed the same wear 'ites characteristics in as little as 500 operating hours.
Additionally, we identified the gear carrier assembly as part number 1A-6114.
Through additional investigation, the subject gear (part number 02-425-03-AB) is
'also part of another gear carrier assembly, part number 1A-6664.
Our letter of October 29, 1991 identified preventative action taken as well as our recommendation for corrective action for the individual sites. This recommended action shall be applicable to assembly part number 1A-6664 as well.
A copy of this letter will be forwarded to the affected sites identified in our original notification.
