Attached is our'eport on the subject item which is provided to you with initial information on an item which we consider to be p'otentially reportable per the provisions of 10CFR21.

Me continue to investigate the observed failure at Harris to determine the root cause, and will use this root cause to determine if an actual Part 21 report is required.

Carolina Power and Light Company considers that the actions taken/planned are adequate for resolution of this item at this time.

ITE Electrical Products 3333 State Bridge Road Alpharetta, Georgia 30201 NATURE OF DEFICIENCY:

These circuit breakers were purchased commercial grade and subjected to thermal and instantaneous trip tests at SHNPP.

The breakers are used in safety-related motor control centers.

Typical loads include the Reactor Coolant System Supports Fans (S-4),

and the Emergency Diesel Generator Room Exhaust Fans (E-86).

SAFETY IMPLICATIONS:

In certain applications, the breaker would not hold the required load resulting in equipment inoperability.

PLAN OF CORRECTIVE ACTION:

1.

A test of the breakers was performed by the vendor on August 22, 1989.

2.

CP&L is reviewing the circuit breaker test procedures used at the plant.

3.

CP&L is identifying safety related applications of these molded case circuit breakers at SHNPP.

4.

Motor in-rush current tests will be performed on the S-4 motor to determine if correct breaker is being utilized, and/or determine motor problems.
