SUBJECT:

Shearon Harris Nuclear Power Plant Unit No. 1 10CFR50.55(e) reportable deficiency. Failure of penetration seal type, B-2, at the Emergency Service Water Intake Structure in Bay 8. The type B-2 seal is also utilired in Bay 6 of the i~ Emergency Service Water Intake Structure, d

ITEM: Penetration seal, type B-2, NNS, non-seismic.

NATURE OF DEFICIENCY: Penetration seal, type B-2 is a water tight seal which utilites a reibber boot design. Stresses imposed on boot materials by raising and lowering-the water level during start-up testing caused failure of the boot. This event caused Emergency Service Water Pump "A" to trip on an over-current signal.

SCOPE OF PROBLEM: The deficiency involves two Unit 1 penetration

seals, type B-2. One in Bay 6 and one in Bay 8 of the Emergency Service Water Intake Structure.

SAFETY IMPLICATIONS: Failure of the non-safety, non-seismic penetration seal could prevent the operation of the Emergency Service Water System by allowing sand to enter the Emergency Service Water Pump bay blocking suction to the pump.

DEFICIENCY IS REPORTABLE: The failure compromised the availability of redundant emergency service water pump to

, supply service water to safety related components. This reduces the degree of j protection to plant systems and to public health.

CORRECTIVE ACTION: A modified penetration closure detail was

. designed and installed in both Bays 6 and 8 of-the Emergency. Service Water Intake Structure.

The detail involved closing the penetration in J

two steps:

Closure of the area inside the wall penetration around the pipe with M-20 grout, i 2. Addition of an external closure plate which

The new detail was designed as Safety Class I.
