Gentlemen:

Subject: Resident Inspection 50-213/86-20

This transmits the findings of the routine NRC inspection of the Haddam Neck Plant

on July 8 through August 14, 1986. These findings have been discussed with

Mr. R. Graves of your staff.

Based upon the results of this inspection, it appears that certain licensee acti-

vities were not conducted in full compliance with NRC requirements. Please respond

to these matters in accordance with the enclosed Appendix A.

With regard to item 2 of Appendix A, we note that corrective action for the two

similar breaches of containment integrity cited in NRC Region I Inspection Reports

50-213/84-14 and 86-08 did not prevent this occurrence. Therefore, in your reply,

please emphasize how the corrective actions taken for this item will prevent fur-

ther recurrence.

We are also concerned that inadequate interface between operators and maintenance

personnel may have resulted in plant operation with equipment not fully operational.

In one case (Report Detail 4.2), operation continued with only one of three emer-

gency boration paths operational because operators apparently failed to take timely

followup action in response to abnormal pump. indications. In the second instance

(Report Detail 4.8), operators apparently failed to recognize that repairs to the

nuclear instrumentation system had changed system performance such that recalibra-

tion was required. In your response to this letter, please include your assessment

of the cause of these events and identify your corrective actions.
