SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-213/98-03
Dear Mr. Mellor:
On August 13,1998, the NRC completed an inspection at Connecticut Yankee Atomic
Power Company. The enclosed report presents the results of that inspection.
During the four month period covered by this inspection period, your conduct of activities
at the Haddam Neck Station facility was generally characterized by the gradual resumption
of radiological work associated with the preparation of procedures and equipment for the
primary system decontamination. In general, the plant staff performed well to safely
complete several maintenance and radiation protection tasks in the sequence prior to the
reactor coolant system decontamination. A notable exampie included the work to remove
the reactor vessel core barrel, which was completed with substantially less personnel
radiation exposure than in the past.
This inspection also noted several events that were exceptions to good performance.
Apparent violations of regulatory requirements were identified, which involved the failure to
classify an event in accordance with the emergency actions levels following an inadvertent
release of radioactive liquids; the failure to control the plant configuration during valve
manipulations or tagging activities, resulting in plant events; the failure to properly calibrate
the stack flow instruments used in the stack effluent pathway; and, the failure to provide
complete information in support of a license amendment application. We are particularly

concerned with the number of operational occurrences that have occurred, and with the

deficiencies that have been identified in surveillance procedures used to satisfy the
technical specification requirements. Therefore, in your response to Violations B and C,
'
you should describe, in detail: (a) the actions to address the underlying causes of plant
I
configuration control and human performance issues; and (b) your plan and schedule to
!
provide an independent verification that the plant procedures for demonstrating compliance
l
with technical specification surveillance requirements are acceptable.

The violations are cited in the enclosed Notice of Violation, and the circumstances
surrounding the violations are described in detail in the enclosed report. Please note that
i
you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with
