REMOVAL OF EXPANSION ANCHORS AFTER INSPECIION ITEM 132 JULY 1, 1983 REPORTABLE UNDER 10CFR50.55(c)

SUBJECT:

Shearon Harris-Nuclear Power Plant Unit No. 1 10CFR50.55(e) reportable deficiency concerning a seismic hanger base plate and expansion anchors being removed after inspection without prior notification to Construction Inspection.

ITEM: DR-C-1860: The hanger base plate installed under anchor placement 1FH261-014 was removed, after installation and inspection, without prior notifi-cation to Construction Inspection as required by procedure.

P SUPPLIED BY: This is not a supplier-related deficiency. The plate was field fabricated and installed.

NATURE OF DEFICIENCY: This plate was installed using drilled-in expansion anchors which were torqued to the required tension pre-load, inspected and accepted. The plate was subsequently removed by craftsmen to facilitate attachment of the seismic hanger, without notification to Construction Inspection as required by procedure to ensure reinspection after reinstallation.

DATE PROBLEM OCCURRED: This problem was detected on April 18, 1983.

DATE PROBLEM REPORTED: On May 3, 1983, Mr. M. M. Pugh notified Mr. A. Hardin, NRC, that this item was potentially reportable per provisions of 10CFR50.55(e). On June 1, 1983, Mr.

K. V. Hate' notified Mr. A. Hardin that this item was reportable.

SCOPE OF PROBLEM: This problem is limited to a single hanger base plate in the Fuel Handling Building.

SAFETY IMPLICATION: The base plate supports a pipe hanger which, in turn, supports the chilled water spray pipe. Had the plate been incorrectly reinstalled and failed as a result, the remaining pipe hangers would not have been adequate to support the pipe.

REASON DEFICIENCY IS REPORTABLE: Engineering evaluation indicates the pipe would not have been adequately supported by remaining hangers had this one failed.

CORRECTIVE ACTION: The hanger base plate has been reinstalled and retorqued and accepted by Construction Inspection.

PREVENTATIVE MEASURE: The inspection procedure TP-39, Rev. 5 (currently in the approval cycle) includes a requirement for an inspection indicator to help ensure that a reoccurrence will be easily detectable.
