December 5, 1975 . Page 2 Designation of Apparent Cause of Occurrence: ,

Broken wire on the closing toraue switch in the valve operator.

Analysis'of Occurrence:

The subject valve .(M0-3-14-26A - Full Flow bypass to Torus) is only open during performance of the Core Spray surveillance test.

The safety implications of this occurrence are minimal because the redundant Core Spray loop was operable and because repairs were completed in a short period of time. Additionally, the "A" loop could have been returned to operability by manually closing the valve.

Corrective Action:

The broken wire was repaired and proper valve operation verified.

Failure Data:

A previous f ailure of this type was reported as Abnormal Occurrence Report Number 50-278-75-51.

Very truly yours,
