Ide'ntification of Ocourrence:
,s The RCIC Inboard Steas Supply Isolation valve (MO-3-13-15) failed to close.
t Conditions Prior to Occurrence:

. Description of Occurrence:
j During the perfornance of a surveillance test, the valve failed to close from the control switch.
The valve vas closed by =anually actuating the elece contactor f.or the valve at the motor control s
center.
The vklve was,then successfully strokei frem the control switch.
The failure was not repeatchie.

Under investigation.
- tAnalysis of Occurrence:
i The safety i=plications of this' occurrence are -ininal because the valve was closed by sanun,11y actuating the close contactor.
Becacce the valve was then succeccrully stroked from the control room, it van 8,
not ceclared incperable.Further, the ontboard isolation valve j
XO-3-13-16 was opernble to perfern an isolation, if required.

To be deternined after. investigation.
