Conditions Prior to Occurrence:
Unit 3 at 56% power.
l Description of Occurrence:
During routine surveillance test, 0 Analyzer Is lation Valve SV 3978D 2
failed to close.
Designatidn of Apparent Cause of Occurrenec:
Under investiCation.
I Analysis of Occurrenec:
i The safety implications of this occurrence are minimal because the i
j inner isolation vclve SV-3671B closed and scated properly when tested.
In addition, the uistream manual isolation valve has been closed to preclude any leakage.
Corrective Action:
To be determined.
