Identification of Occurrence:
Failure of LS-5223 to operate SV-5223 (RHR head spray stay fill vent)
I Conditions Prior to Occurrence Cold shutdown.
Description of Occurrence :
During a routine instrument aurveillance test.

LS-5223 was found'to Designation of Apparent Cause'of Occurrence:
Binding in the switch flo,at assembly.
Analysis of Occurrence.
The head spray line was full prior to the test, and the reactor was in the cold shutdown condition.
This occurrence has minimal safety y
implications.
. Corrective Action:

. The stay fill tank was refilled.
The switch will be repaired.
Failure Data:
Previous failu'res reported as 50-277-75-4 and 50-278-75.
