Fa'ilure of LS-4223 to operate SV-4223 (RHR Head Spray Line High Point Vent) on low accumulator level.

Conditions Prior to Occurrence:

Unit 2 shutdown.

Description of Occurrence:

Upon receipt of the head spray accumulator low icvel alarm, investigation revealed that the vent solenoid valve f ailed to open to refill the head spray high point accumulator.

e The fuse in the solenoid valve circuit was found blown.

Investigation revealed a ground in the solenoid valve circuit. The ground'ias corrected and the accumulator returned to normal service.

Analysis of Occurrence:

There are no safety implications because the Icvel switch alarmed properly and the operator was abic to manually perform the function of the vent solenoid valve.

Corrective Action:

The operator manually vented the accumulator to ensure that the head spray line was maintained filled. The head spray accumulator low icvel alarm was operabic at all time. Until the time when the accumulator vent valve was returned to service, the accumulator could be manually vented. The grounded wire was repaired and the vent valve returned to service.

Similar f ailures reported as A0-50-277-75-4, 50-278-75-20, 50-277-75-34, 50-277-75-44 and 50-278-75-62.
