1 Identification of Occurrence:
Closed upper isolation valve on LS-2-3-231C...

Conditions Prior tc Occurrence:
v.
Unit 2 operating at 551 power.
Description of Occurrence:
During routine instrument surveillance test, the upper isolation valve to LS-2-3-231C (Scram Discharge Volume Hi Level Scram) was found closed.
Designation of Apparent Cause o.f Occurrence:
The most probabic cause for this occurrence is technician error in returning these instruments to service after the previous surveillsace test.

y Failure of a single switch to function in a one-of-two-tvice logic systes does not render the logic syste= inoperable.
Scram on high scras discharge volume level vould still have occurred with this switch inoperable.
Therefore, this occurrence has no safety implications b
Corrective Action:
The technicians involved were interviewed and the seriousness of the

error stressed.

Yalving errors on instruments covered by surveillance testing have been reported as AO-50-277-Th-36. 50-278-TL-10 50-277-75-6 and 50-271-75-21.
