Designation of Apparent Cause of Occurrence:
Flow blockage in the on stream sampic line (one of seven availabic sampic lines).
i Analysis of Occurrence!
The flow blockage occurred in only one of the seven availabic sampic points and although there was temporarily no flow through the Drywell Radiation Monitor, it was still capable of operation with any of the remaining 6 (six) sample points. When flow was re-established' I
l via one of the available 6 points, drywell radiation levels were as expected.
In addition, the Drywell Floor Drain Sump remained operabic through out this period with the indicated flow normal. Therefore, the safety implications of this occurrence are minimal.
r Corrective Action:
The sampic point was changed to another point to re-establish sample flow to the Drywell Radiation Monitor. Corrective action awaits 3
l an appropriate opportunity.
1 Failure Data:
Similar occurrences were reported as Abnormal 0ccurrence Reports 50-278-75-65 and 50-278-75-74.
i Very truly yours,
]
