Identification of Occurrence:

d Failure of the main stack flow transmitters.

. Conditions Prior to Occurrences i,

Unit 2 at about 555 power, Unit 3 shutdown.

Des'eription of Occurrence:

On the night of September 20, 1975, a lightning strike disabled both main' stack flow transmitters.

  • Designation of Apparent Cause of Occurrence:

Lightning strike.

l Analysis of Occurrence:

Flow rate prior to the strike was known and was being supplied by two i

dilution fans.

These fans rensined on during the short time that the f

transmitters were out of service, so total stack flow during this perioc is known.' This occurrence has minimal safety implications.

Corrective Action:

l The damaged conponents were replaced and the transmitters were returned to service.

Previous failures of this type were reported as 50-277-75-51, 50-278-75-38, 50-277-75-5k, and 50-278-75 hk.
