Conditions Prior to Occurrence:

Reactor at full power steady state.

Deccription of Occurrence:

During normal steady state operation, the 'C' refueling area exhaust radiation aceitor failed dovnocale.

Designation or Apparent Cac.se of Occurrence The apparent cauan of the occurrence is under investigation at this time.

Analysis of Occurrence Because this device is one of four arranged in a one-out-of-two twice logic (which initiates a secondary containant ventilation cystem isolation on refueling floor exhaust high radiation) such that the loss of any one device vill not defeat t',e logic, the safety imp 1*. cations are minimal.

Corrective Action:

This channel was i::::codiotely placed in the tripped condition in coc:pliance with Table 3.2.D of the Technical Specifications.
