4. Occurrence Date: 15 April 1977
5. Identification of occurrence:

Chemical Addition Valve CAV-2 and Make-Up Valve MUV-258 inoperable contrary to Technical Specification 3.6.3.1.

6. Conditions Prior to Occurrence:

Normal Mode 1 operation.

7. Description of Occurrence: -

At 0600 while performing the quarterly cycling of power-operated con-tainment isolation valves, Surveillance Procedure SP-370, CAV-2 and MUV-258 failed to close from the Control Center. Redundant isolation valves were available and operable. MUV-258 was restored operable within 4

hours. CAV-2 was manually isolated and will be repaired at the next scheduled outage. ,

8. Designation of Apparent Cause of Occurrence:

Corr.oded and dirty contacts for MUV-258. Closure spring for CAV-2 not  ;

strong enough to close valve.

)

9. Analysis of Occurrence:

There were no safety implications because of this occurrence as there were redundant valves available and operable.

1 10. Corrective Action:

Contacts .ior MUV-258 were cleaned and valve restored operable. Preventive Maintenar :e Program should prevent recurrence. Closure spring for CAV-2 will.be i inewed on next scheduled outage.

11. Failure Data:

This was a non-repetitive occurrence.
