50-302 /77-103E Facility:

Crystal River Unit #3 Report Date:

9 September 1977

~

Occurrence Date:

3 September 1977 Identification of Occurrence:

Unplanned gu release reported in accordance eith ETS 5.6.2.(A).

Conditions Prior to Occurrence:

Normal power operation. Prepering to vent Makeup Tank via low pressure Waste Gas Header..

Description of Occurrence:

At 0716 upon venting the Makeup Tank to the low pressure Waste Gas Eeader, airborne radiation monitors RMA & RNA-3 alarmed ' actuating appropriate ventilation trips.

Subsequent investigation revealed that the source of the release was the loop seal on the miscellaneous Waste Evaporator. Loop seal was re-established upon discovery.

The maximum release' rate at the peak of the discharge profile was 0.53% of the most restrictive limit set forth in Environmental Technical Specification 2.4.2.A(1).

System modifications to prevent occurrences of this nature have been initiated and -

submitted to Production Engineering.

Designation of Apparent Cause:

Pressure in the low pressure Waste Gas Header exceeded loop seal capability.

Analysis of Occurrence:

There was no consequence to the health or safety of the plant or public because of the very small release rate.

Corrective Action:

Proposed modifications noted in the Description of Occurrence should preclude recur-rence of this nature.

[

Pailure Data:

This has been a repetitive problem.
