Occurrence Date:

Identification of Occurrence:
Containment isolation valve SW-47 inoperable contrary to Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:
Mode 1 power operation.
7.
Description of Occurrence:
At 0700, while performing the quarterly valve cycling surveillance procedure, SP-370, Nuclear Services Closed Cycle Cooling Valve SW-47 would not close from the Control s.
Room.
Redundant valve SW-25 was manually closed and valve SW-47 was taken out-of-service for maintenance. Maintenance was performed restoring SW-47 operable.

Designation of Apparent Cause:
This event was caused by a loose wire on the limit switch.
9.
Analysis of Occurrence:
There were no safety implications due to this event, as redundant valve SW-25 was utilized for containment isolation.
10.
Corrective Action:
None required as normal maintensace restored operability of SW-47.
11.
Failure Data:
This was the first occurrence of SW-47 failure to operate.
