Containment isolation valve WDV-94 was inoperable contrary to Technical Specification 3.6.3.1.
- 6. Conditions Prior to Occurrence:
Mode 1 power operation.
- 7. Description of Occurrence:

At 0700, while performing the quarterly cycling of valves Surveillance Procedure 5-SP-370, valve WDV-94 failed to operate. The 4 hour4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> action statement was entered.
Maintenance was performed and valve restored operable within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />.
- 8. Designation of Apparent Cause:
This event was due to the limit switch contacts being dirty. The contacts were cleaned and valve restored operable.
- 9. Analysis of Occurrence:
There was no safety implications due to this event, as a redundant waste disposal valve was used to isolate the system.
- 10. Corrective Action:
None required as normal maintenance restored the valve's operability.
- 11. Failure Data:
This was the first time valve WDV-94 had failed to operate..
