4. Occurrence: 6 April 1977
5. Identification of occurrence:

In the process of moving Group 8, Axial Power Shaping Rods (APSR's),

the absolute position indicaor for Rod 5 failed causing a condition contrary to Technical Specification 3.1.3.3.

6. Conditions Prior to Occurrence:

Normal Mode I operation. -

7. Description of Occurrence: 2 At 0812, while adjusting a power imbalance wit! Group 8 Rods, absolute pcsition indicator for Rod 5 failed. Rod posi*. ion was verified by the Rod Zone Limit Switch. Commenced logging position agreement between r reference indicator and the operable position indication as per SP-300.

l ( Position indicator for Rod 5 was restored operable during shutdown on 8 April 1977.

8. Designation of Apparent Cause of Occurrence:

The amphenol connector to the position indicator was found to have a faulty pin connection.

9. Analysis of Occurrence:

There were no safety implications as the Reactor could have been safely shut down at any time.

10. Corrective Action:

The amphenol connector was repaired and absolute position indicator for Rod 5 of Group 8 was restored operable.

11. Failure Data:

This occurrence wat non-repetitive for the APSR group.
