Chicago, Indiana     46312 NATURE OF DEFECT:     During the welding of the support plates, a large lamination in one plate was noticed. A closer inspection of plates previously welded showed smaller laminations.
DATE PROBLEM WAS CONFIRMED TO EXIST: January 17, 1980 PROBLEM REPORTED: January 17, 1980-CP&L (L.E.Jones and A.M.Lucas)informed the NRC Region II office (Mr.R.Bradley)by telephone that the problem was"potentially reportable" under 10CFR50.55(e) and 10CFR21.February 14, 1980-CP&L (N.J.Chiangi)informed the NRC Region II office (Mr.M.Hunt)by telephone that the problem was reportable under 10CFR50.55(e) requirements.

N SCOPE OF PROBL&i: One plate from Graver C-27 heat no.802J06219 was used to fabricate the maintenance rail support plates.While welding one plate, a large lamination occurred.SAFETY LMLICATIONS:
February 14, 1980 - CP&L (N. J. Chiangi) informed the NRC Region   II office (Mr. M. Hunt) by telephone that the problem was reportable under 10CFR50.55(e) requirements.                               N SCOPE OF PROBL&i:   One plate from Graver C-27 heat no. 802J06219 was used to   fabricate the maintenance rail support plates.
The maintenance rail is supported from the dome.If the rail were to fall, there would be a good chance that it would damage a piece or pieces of safety related equipment.
While welding one plate, a large lamination occurred.
REASON(S)PROBLEM WAS REPORTABLE:
SAFETY LMLICATIONS: The       maintenance   rail is   supported from the dome.
A support plate having laminations does not meet its intended design.This condition is addressed in 10CFR50.55(e) and is a condition which"...were it to have remain uncorrected, could have affected adversely the safety of operations...and which represents: (iii)a significant deficiency in construction...
