O'Reilly LER No.83-005, Revision 1 Page Two February 22, 1983 DETAILED DESCRIPTION OF EVENT On January 12, 1983, at 1440 hours0.0167 days <br />0.4 hours <br />0.00238 weeks <br />5.4792e-4 months <br />, with the Plant in Mode 1, Fire Detection Zone DDD in the Service Water Pumphouse (Room 41-01) failed to reset during the performance of the operational test identified in Technical Specification Surveillance Requirements 4.3.3.7.1 and 4.3.3.7.2. An hourly fire watch patrol was established within one (1) hour of the occurrence in compliance with Action Statement (a) of Technical Specification 3.3.3.7.
PROBABLE CONSEQUENCES There were no adverse consequences from this event since the functional portion of the Fire Detection Instrumentation Surveillance Test Procedures is designed to detect system abnormalities upon the conclusion of field testing. The establishment of the hourly fire watch patrol provided assurance that any fire would have been promptly discovered and extinguished.
CAUSE(S) OF THE OCCURRENCE The cause of the failure to reset was a grounded smoke detector.
The detector head had not been properly installed on the detector base after the completion of the previcusly performed sensitivity test.
IMMEDIATE CORRECTIVE ACiiONS TAKEN An hourly fire watch patrol was established within one (1) hour of the occurrence. The cause of failure was discovered to be a short to ground on Smoke Detector IXA-4985F. The detector head had not been properly installed on the detector base after the completion of the previously performed sensitivity test. Fire Detector Zone DDD was declared operable on January 19, 1983, at 1100 hours0.0127 days <br />0.306 hours <br />0.00182 weeks <br />4.1855e-4 months <br />, upon the satisfactory completion of an operational test performed with the appropriate surveillance test procedure.
ACTION TAKEN TO PREVENT RECURRENCE Test personnel have been cautioned to properly install smoke detector heads so as to prevent future occurrences of a similar nature.
