Mr. James P. O'Reilly LER No.83-023 Page Two April 8, 1983 EVENT DESCRIPTION AND PROBABLE CONSEQUENCES

! On March 9,1983, with the Plant in Mode 1, the Main Control Board pressure indication for Reactor Building Pressure Channel 950

, failed low. The instrumentation channel was declared inoperable at 0300 hours0.00347 days
0.0833 hours
4.960317e-4 weeks
1.1415e-4 months
and subsequently placed in the bypassed condition 1

within one (1) hour in compliance with Action Statement 16 of Technical Specification 3.3.2, Table 3.3-3, Items 2.c and 3.b.2.

4 Similar failures have occurred to this instrumentation channel on December 2, 1982 (LER 82-016) and February 3,1983 (LER 83-009).

All of the failures are attributed to the same component failure.

There were no adverse consequences during any of' these events since.the remaining three (3) channels of Reactor Building Pressure remained operable.

CAUSE AND CORRECTIVE ACTIONS The cause of the events is attributed to an intermittent electronic failure of Reactor Building Presssee Transmitter PT-950. Af ter the channel was placed in the bypassed condition on March 9,1983, the circuitry returned to normal operation as previously documented in the earlier failures. The channel was returned to operable status at 1000 hours0.0116 days
0.278 hours
0.00165 weeks
3.805e-4 months
on March 9,1983, after the completion of-a satisfactory surveillance test. The licensee is presently in the process of replacing Reactor Building Pressure Transmitter PT-950 and expects this replacement will prevent a repeat of the previous failures. Since the Plant is presently in an outage, the transmitter replacement is expected to be complete prior to entry into Mode 3.
