O'Reilly LER No.83-018 Page Two March 25, 1983 EVENT DESCRIPTION AND PROBABLE CONSEQUENCES At 1410 hours0.0163 days <br />0.392 hours <br />0.00233 weeks <br />5.36505e-4 months <br /> on March 6, 1983, with the Plant in Mode 1, Nuclear Sampling System Valves 9398 A, B, and C failed to close during the initial performance of the accelerated teet program established to exercise these valves on a weekly basis'(Reference LER 83-011,

" Action Taken to Prevent Recurrence") to insure their continued operability in accordance with Technical Specification 3.6.4.

Valves 9398 A and B were returned to operable status by 1645 hours0.019 days <br />0.457 hours <br />0.00272 weeks <br />6.259225e-4 months <br /> in compliance with Action Statement (a), whereas Valve 9398C was isolated in accordance with Action Statement (c) until repairs could be performed.

No adverse consequences resulted from this event since the compliance with the Technical Specification Action Statements minimized the possibility of a loss of containment integrity.

CAUSE AND CORRECTIVE ACTIONS Nuclear Sampling System Valves 9398 A, B, and C have experienced a recurring stroke problem because of crud buildup on the valve seats. The valve stroke was returned to normal by 1645 hours0.019 days <br />0.457 hours <br />0.00272 weeks <br />6.259225e-4 months <br /> on all of the valves when the crud was dislodged by means of tapping on the valve body. Valve 9398C was not returned to operable status at this time, however, because of a failure of the valve closed position indication. The manual isolation valve associated with Valve 9398C was subsequently closed at 1700 hours0.0197 days <br />0.472 hours <br />0.00281 weeks <br />6.4685e-4 months <br />.

The valve position indication f ailure of Valve 9398C was found to be a faulty limit switch. The limit switch was replaced, and the valve return 2d to operable status af ter the completion of a satisfactory surveillance test at 1130 hours0.0131 days <br />0.314 hours <br />0.00187 weeks <br />4.29965e-4 months <br /> on March 9, 1983.

ACTION TAKEN TO PREVENT RECURRENCE The following actions are being taken by the licensee:

1. The Plant is attempting to procure replacement valves more suitable for the application as discussed in LER 83-011.
2. This event occurred on the initial implementation of the accelerated test program. The next test (performed on March 14, 1983) was acceptable and indicates that the valves will remain operable, until replaced, with the performance of the weekly test.
