= August 10, 1990 Page 1 of 2 RESPONSE TO NOTICE Of VIOLATION VIOLATION NUMBER 50-395/90-18-01 I. ' ADMISSION OR DENIAL OF VIOLATION South Carolina Electric & Gas Company (SCE&G) is in agreement with the ,

alleged violation.

II. REASON FOR THE VIOLATION The reason f or the violation is contributed to. personnel error, in that  :

a valve was manipulated by-the operators outside of the system alignment which led to the isolation of chill water flow to the 'B' Component Cooling Water (CCW) pump motor. Although the major contributing factor to this event was a failure to adequately implement procedures, the labeling and proper identification of the components to be manipulated was also identified as a contributor. The misalignment of a valve providing cooling water to the "B" CCW pump motor did not result in a loss of the capability of the system to perform its design function.

Therefore, this event had no impact on safety.

III. CORRECTIVE STEPS TAKEN AND RESULTS ACHIEVED The individuals involved in this event were counseled on procedural compliance and the importance of independent verification in maintaining proper system alignment. In addition, Operations management met with i all operating shift personnel to stress the requirements of independent l verification and procedural compliance. The counseling ano the meetings l have elevated the level of sensitivity to.these issues for the operating shifts and have reinforced managements level of concern as to the importance of procedural compliance and independent verification.

g '. . nAttachment l'to Document Control Letter August 10, 1990 Page 2 of 2 IV. CORRECTIVE STEPS TO BE TAKEN TO AV010 FURTHER VIOLATIONS AND DATE OF FVLL COMPL1AMCE 1.. The inlet and outlet valves to both trains of cooling water provided to the CCW pump motors will be entered into the locked valve program. These are normally open valves and only require manipulation for removal of components from service under the danger tag program.

Completion date August 17. 1990

2. Valves which are manipulated frequently for the purpose of surveillance testing or system realignments will be relabeled to provide a clearer identification of the component, immediate action is being taken to identify chill water valves associated with component cooling water system to facilitate relabeling as soon as possible.

Completion date December 15. 1990

3. The Associate Manager, Operations initiated a lineup reverification of 8 Train Reactor Building Spray and Emergency feedwater Systems to .l ensure that the valve misalignments were not generic to all system lineups. These lineups were satisfactory with no additional discrepancies identified.

4. Station procedures are being reviewed to verify that expectations for independent verification are clearly defined and the appropriate personnel are trained to the procedural requirements.
