Mr. James P. O'Reilly LER No.83-015 Page Two March 17, 1983 DETAILED DESCRIPTION OF EVENT On February 19, 1983, with the Plant in Mode 1, a discrepancy of 5%
was observed between the Main Control Board (MCB) and Control Room Evacuation Panel (CREP) indication for Pressurizer Relief Tank Level. The Channel check, which discovered the discrepancy, was being performed in compliance with the increased surveillance frequency identified in Licensee Event Report #83-007, Revision 1.
The CREP indication (LI-470A) was declared inoperable in accordance with Action Statement (a) of Technical Specification 3.3.3.5.
PROBABLE CONSEQUENCES There were no adverse consequences from this event. The level indication was functional but drifted low during the period of time between January 25 and February 19, 1983.
CAUSE(S) OF THE OCCURRENCE The cause of this occurrence is attributed to design error. The recurring zero shift observed on level transmitter LT-470A was due to operation beyond the manufacturer's design specifications for this type transmitter.
IMMEDIATE CORRECTIVE ACTIONS TAKEN A calibration performed on the CREP indication channel for Pressurizer Relief Tank Level showed that the transmitter (LT-470A) had experienced a zero shift in output. Additional investigation performed by maintenance personnel revealed that the zero shift was occurring because the installed transmitter did not have the correct design parameters (operational range) for the plant application. The transmitter was replaced with a model that had the correct operating range, and the CREP indication channel was returned to operational status on February 24, 1983, upon the completion of an acceptable Channel Check.
ACTION TAKEN TO PREVENT RECURRENCE The licensee considers the correction to the system design as adequate action to prevent a future recurrence of this problem.
