During a routine instrument surveillance test, dPIS-2-2-1290 failed to respond to a differential pressure signal.

Deninnation of Anparent Cause of Occurrence:

Linkage binding caused by wiring within the switch.

Analysis of Occurrence:

'Ihe safety implications of this failure are mininal because this switch is part of a one-of-two twice logic and would not have preventoi to correct loop selection.

Corrective Action:

Upon disassembly of the switch, it was found that one of the intemal connecting wires vos interfering with the switch linkage.

The viring was redressed; the switch was recalibrated and tested in acconlance with the applicable surveillance test.
