REPORT DATE: January 9, 1975 OCCURRENCE DATE: December 30, 1974 FACILITY: Quad-Cities Nuclear Power Station Cordova, IL 61242 IDENTIFICATION OF OCCURRENCE:

The setpoint of TS-2-261-17C, in the Primary Containment isolation System, was found to exceed its Technical Specification limit.

CONDITIONS PRIOR TO THE OCCURRENCE:

The unit was in cold shutdown for a refueling outage.

DESCRIPTION OF OCCURRENCE:

On December 30,1974 at 1100 hours0.0127 days <br />0.306 hours <br />0.00182 weeks <br />4.1855e-4 months <br />, while doing routine REFUELING OUTAGE sur-veillance, TS-2-261-17C was found to trip at 222 F. This was in violation of Technical Specification 3.2.A. No operator actions were needed to bring the ~

situation under control.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

O_ther - Instrument Drift The apparent cause of this occurrence is instrument drift. The switch was replaced in June of 1974, along with the other fif teen switches, because of moisture problems in the conduits. These problems have since been corrected.

The "as-lef t" value at that time was 182 0F.

ANALYSIS OF OCCURRENCE:

Each Primary Containment isolation sub-channel has four of the temperature switches wired in series. Since all setpoints of the other switches were within limits the system would have functioned as designed if needed.

Therefore, there are no safety implications resulting from this occurrence.

CORRECTIVE ACTION:

The switch was recalibrated and installed back into .the system. All switches

- were functionally tested satisfactorily. The use of alternate suppliers for these temperature switches is being. evaluated.
